With VillageCareMAX, you can maintain your doctor while Medicare or Medicaid continues covering hospital visits, emergency care, and prescriptions, depending on your coverage.

- A health plan offering long-term care services covered by Medicaid.
- Replaces Medicaid fee-for-service by managing your health care needs.
- A Care Manager and team, including a Registered Nurse, work with you to coordinate all care, even those outside the plan’s package.

- 18+ years old.
- Medicaid eligible.
- May need nursing home eligibility at enrollment.
- Must be able to live at home or in the community.
- Expected to require long-term care services for more than 120 days.
