Senior Health Partners MLTC Medicaid Plan
The Senior Health Partners Managed Long-Term Care Medicaid Plan by Healthfirst offers a comprehensive approach to care, delivering essential long-term care services such as personal care assistance and adult day healthcare. You will also benefit from a dedicated Care Team led by a Registered Nurse or Licensed Social Worker. This team’s primary objectives are to
(1) collaboratively design a personalized care plan tailored to your specific needs and goals and
(2) efficiently coordinate the range of services required to support your well-being and independence.
If you already have medical and prescription drug coverage through a Medicare plan and require additional long-term care services, our integrated plan offers even more advantages, including an extra $250 monthly allowance for over-the-counter medications, nutritious food options, and essential home utilities. You may be eligible for Senior Health Partners if you:
- Are 18 years of age or older
- Reside within New York City’s five boroughs (The Bronx, Brooklyn, Manhattan, Queens, and Staten Island), or in Nassau or Westchester counties.
- Are Medicaid eligible
We work with Healthfirst and the caregiver received the following from us
- Caregivers receive a competitive rate of $21.09 per hour.
- For every 30 hours worked, caregivers accumulate 1 hour of paid sick leave, with the ability to save up to 56 hours of sick leave in a year.
- This sick leave is also paid at the same rate of $21.09 per hour.
- Additionally, caregivers receive their pay on a weekly basis through direct deposit.
