Fidelis Care at Home Program
Fidelis Care at Home is a program designed for individuals who require long-term care services and assistance to maintain their independence while residing in their own homes and communities.
A diverse array of services is accessible to cater to individuals based on their unique situations. These services encompass nursing care, home health aides, therapy, medical supplies, adult day social care, and more.
Who is eligible to apply?
If you meet the following criteria, you may qualify for Fidelis Care at Home:
- You are currently enrolled in or eligible for New York State Medicaid.
- You are at least 18 years old.
- You have undergone an assessment by a nurse and have been determined to require the necessary level of care.
Availability
Enrollment in Fidelis Care at Home is open throughout the year.
Advantages
As a member of Fidelis Care at Home, you will collaborate with your physician and Nurse Care Manager to develop a customized care plan that aligns with your specific needs. This plan can encompass a wide range of covered services, including:
- Home healthcare, which includes nursing, physical therapy, and/or occupational therapy.
- Personal care assistance for daily living tasks.
- In Home Care Services
- Nutritional services.
- Private day nursing.
At Health Aide Inc, we partner with Fidelis to provide in-home care services. If you are enrolled with Fidelis and require in-home care, we are here to assist you. Our offerings to caregivers include:
- Competitive hourly rate: Caregivers receive a competitive rate of $21.09 per hour.
- Paid sick leave: For every 30 hours worked, caregivers accrue 1 hour of paid sick leave. You can accumulate up to 56 hours of sick leave in a year. This sick leave is also compensated at the same rate of $21.09 per hour.
- Weekly pay: Caregivers receive their pay on a weekly basis through direct deposit for added convenience.
- Support and assistance: If caregivers require assistance or have any questions, they can easily schedule a call with us through our calendar.
