Empire BlueCross BlueShield HealthPlus Managed Long Term Care Plan, is dedicated to providing comprehensive home care and family caregiver services for adults living with long-term health conditions or disabilities.

At Empire Managed Long-Term Care (MLTC), their primary objective is to enable their members to maintain their independence and well-being in the comfort of their own homes. They recognize that receiving care at home is often the preferred choice for many individuals, and they are there to support individuals in achieving that goal. One of the key features of their program is the Consumer Directed Personal Assistance Service (CDPAS), which empowers members to have more control over their care.
With CDPAS, you have the freedom to choose your personal assistant, and this can include family members or friends who meet specific criteria. This flexibility empowers you to build a care team that you trust and feel comfortable with. Your personal assistant can assist you with a wide range of daily activities, such as bathing, dressing, meal preparation, toileting, walking, and feeding. Additionally, you can receive skilled services like tube feeding, medication management, wound care, and respiratory care through this program.
Here’s how the CDPAS works:
Choice and Control:
You or a person you designate are responsible for selecting, hiring, training, and managing your personal assistant. This means you have the autonomy to make decisions regarding your care.
Responsibilities:
Your responsibilities also include managing your personal assistant’s schedule, ensuring their timesheet is accurate, and approving it. You can make changes to your personal assistant if needed.
Registration with Fiscal Intermediaries:
When you choose a personal assistant, they will need to register with us, Health Aide is one of our partners. We “Health Aide” will handle the payment for your personal assistant based on the hours they work.
Flexibility:
If you ever decide that you no longer wish to participate in the self-directed CDPAS program, you can transition to receive regular personal care services through Empire MLTC at any time.
What We Offer To Caregivers:
- Caregivers receive a competitive rate of $21.09 per hour.
- For every 30 hours worked, caregivers accumulate 1 hour of paid sick leave.
- Caregivers can save up to 56 hours of sick leave in a year.
- Sick leave is paid at the same rate of $21.09 per hour.
- Caregivers receive their pay on a weekly basis through direct deposit.
- For assistance or questions, caregivers can easily schedule a call on our calendar.
Check your eligibility
To be eligible for CDPAS, you should meet the following criteria:
- Demonstrate the ability to make informed decisions regarding your care.
- Be capable of conducting interviews, providing training, giving directives, and more.
- Maintain a record of your personal assistant's work hours.
- Submit timesheets to one of Empire's fiscal intermediaries (partners) to ensure your personal assistant receives their payment.
